Job Description:
A company is looking for a Social Determinants of Health
Coordinator.
Key Responsibilities
Maintain and manage an updated resource list for community-based
services
Coordinate referrals for members to community services and
programs
Conduct outreach and provide resources to enrollees not in care
management
Required Qualifications
2+ years of experience with community resources or social service
agencies
1 year of professional work experience
Intermediate knowledge of Microsoft Office programs
Ability to work in a fast-paced environment
Must reside in Louisiana and work 40 hours a week, Monday through
Friday
