Palliative Care Navigator Job DescriptionChristianaCare is seeking a skilled Palliative Care Navigator to join our team in Newark, Delaware. As a Palliative Care Navigator, you will play a vital role in coordinating care for patients and their families, ensuring they receive the best possible support and treatment.Key Responsibilities:

• Coordinate referrals from within and outside of Christiana Health Care
• Assist Palliative Care staff with coordinating care and facilitating referrals
• Identify potential and realized barriers to care and facilitate referrals
• Schedule and/or facilitate scheduling of appointments
• Triage of family/caregiver phone calls and messages
• Coordinate response with Palliative clinicians based on clinical assessment
• Organize inpatient palliative census and daily workload
• Evaluate and triage consults by collaboration with the hospital teamRequirements:
  • BSN required
  • Masters in nursing preferred
  • RN license or eligible for licensure in the state of Delaware
  • Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferredAbout ChristianaCare:Delivering health, not just healthcare, is our promise to our community.
