ChristianaCare is currently seeking a full-time Registered Nurse
Coordinator for our outpatient Heart Failure practice. The primary
location for this role will be at the Heart and Vascular facility
on the Newark, DE campus. This position will also support our
practice located on the Wilmington, DE campus approximately 2

TeamIn this position, you will work with a talented team of 4
physicians, 4 advanced practice clinicians, and 3 other Registered
Nurses, along with other committed staff to support our heart
failure patients.Work Schedule:Monday-Friday , day shiftPrincipal
Duties and Responsibilities:
- Clinical RN duties
- Provide in person and telephone-based counseling and education
to Advanced Heart Failure and LVAD patients and families.
- Educates patients regarding health and illness prevention.
Recommends community resources to meet patient and family needs.

- Implements and monitors care plans. Monitors, records and
communicates patient condition as appropriate.
- Establish educational materials and methods to assess patients
baseline knowledge and health and evaluate progress towards patient
goals.
- Coordinates care with home health and other providers as
needed.
- Performs phlebotomy and delivers IV diuretic therapy on heart
failure patients as ordered by heart failure provider in the
outpatient diuretic clinic.
- Triages patient and family phone calls and provides
consultation with Physicians and Nurse practitioners.
- Monitor patients receiving IV Diuretic Therapy in the clinic
and update physicians regarding patient statuses in person or via
telephone.
- Perform ReDsvest measurements on heart failure patients in both
the inpatient and outpatient setting.
- Oversee clinical work of the Medical Assistants in outpatient
clinic ensuring appropriate pre-provider visit evaluation is
complete and accurate.
- Adjusts patient medications and review lab results following
CHF/LVAD medication protocols in collaboration with physician or
and nurse practitioners.
- Monitors patients with CardioMems implants.
- Works with patients selected for the Heart Failure Re-Admission
Reduction Program (Task Force), meeting with them to introduce the
program, perform ReDsvest measurements, coordinate care between the
primary team and the Task Force multidisciplinary team and ensure
discharge planning is coordinated based off of recommendations from
the Task Force multidisciplinary team.
- Supports the heart failure disease management program to
achieve established goals and program objectives
- Other duties as assigned to support the advanced heart failure
and LVAD programs.Education and Experience Requirements:
- BSN, graduate of an accredited school of nursing
- 3 years clinical experience in cardiac care/critical care
required
- Heart failure experience is preferred.
- BLS certification, ACLS required.
- an equivalent combination of education and experience may be
substituted.Special Requirements:
- Current license to practice nursing in the State of
